NOTE: The reporting of Security incidents via the HIRO system is limited to staff within Security Services only. If the incident you wish to report does not have a health and safety aspect, please contact Security Services directly. If the incident you wish to report is primarily of a health and safety nature but also has security implications, please report this incident using HIRO and select "yes" to the security implications section.

How can I select a UTS staff member as a witness to an incident?
You can enter the details of UTS staff that witnessed an incident in two ways.
- Type their name and details in the text box, or
- Click on the Search (magnifying glass) icon and search for them.
You can add multiple witnesses.
Note when entering details of stolen or damaged items
Please note that "Asset" refers to University assets and "Personal" relates to all other non-University items.
Note regarding "External Action"
Any theft or loss of University property needs to be reported to the Police by the person to whom this incident occurred.
What sort of security incidents have health and safety implications?
A security incident may have health and safety implications if it has resulted in the risk of (or actual):
- serious personal injury
- serious burns
- loss of consciousness caused by physical impact, exposure to chemicals, electric shock or lack of oxygen
- placing of a person on a life support system
- major damage to any plant, equipment, building or structure
- an uncontrolled explosion or fire
- an uncontrolled escape of gas, chemicals or steam
- a chemical spill
- entrapment of a person in a confined space or in machinery
- collapse of an excavation.
