What are the implications of incident reporting?

Once submitted, the details of incidents reported using this system may become a legal document. Please ensure that you report a factual and reliable account of the situation.

How do I specify the on-campus location where an incident occurred?

Select the On Campus button and then click the Search Location (magnifying glass) icon. Then click on the downward-pointing arrow icon to select the campus, click again to select the building, then the floor and finally the room.

How do I specify the location of an on-campus incident which isn't listed in the Find Location window?

Enter as much information as possible - such as the Campus, Building or Level - in the Find Location window. Click the Select button, then enter further details in the Location Details text box to identify the location.

How do I enter the details of the UTS staff member who sustained this incident?

From the drop-down menu, select UTS Staff, then click the Search (magnifying glass) icon. Search for the staff member then click on their name to select them.

How can I select a UTS staff member as a witness to an incident?

You can enter the details of UTS staff that witnessed an incident in two ways.

  • Type their name and details in the text box, or
  • Click on the Search (magnifying glass) icon and search for them.

You can add multiple witnesses.

What sort of incidents have security impacts?

This incident may have a security impact if it has resulted in:

  • Assault, sexual assault, indecent assault or act of indecency
  • Robbery or theft
  • Disorderly conduct, trespass, offensive conduct/language
  • Harassment, threatening behaviour or private nuisance
  • Accidental or malicious property damage, vandalism, graffiti
  • Drug possession, use, dealing or trafficking
  • Person/s escorted from University premises
  • Fire alarms (false alarms, actual alarms, building evacuations)
  • Security breach (area found unsecured, unauthorised access, PIN abuse)
  • Lost property

What will happen if I choose "yes" to the question, "Did this incident have an impact on security?"

In addition to the person to whom you assign this incident for follow-up, if you choose "yes" to the question, "Did this incident have an impact on security?" then UTS Security Services will also be notified of this incident.

Whose name should appear in the "Assign to:" field?

For students, the person whose name appears in the Assign To: field is the default role for your Faculty. Unless you know the name of the UTS staff member to whom this incident should be assigned, please do not change this setting.


For staff, this is the supervisor of your primary UTS role. If you need to report this incident to a different supervisor, select the Search (magnifying glass) icon to search for and select the appropriate person.

How can I enter details of UTS staff that I've notified of an incident?

You can enter the details of UTS staff that you've notified of an incident in two ways.

  • Type their name and details in the text box, or
  • Click on the Search (magnifying glass) icon and search for them.

What limits apply to file attachments?

When attaching files to hazard or incident reports, please note that a size limit of 4 MB per file applies.
