In the event of a serious incident

If you answer "yes" to any of the items under the "Did any of the following occur?" heading, please phone the Safety & Wellbeing branch immediately.

Important note on staff consultation

It is the responsibility of the immediate supervisor or person responsible for the work area to investigate hazards and incidents reported to them. Any corrective actions must be planned and implemented in consultation with the staff member/s affected.

Rating the level of risk

After putting these controls in place, what will be the level of risk? Please note the likelihood of an incident occurring and the consequence of the incident. Aim to make the risk low if possible.

Explanation of risk levels

  • Extreme - Immediate action required, Senior management action required.
  • High - Senior management attention needed and management responsibility specified.
  • Medium - Manage by specific monitoring or response procedures, Put treatments in place.
  • Low - Manage by routine procedures, unlikely to need specific application of resources.

How can I amend an Action Plan?

From the Welcome page select either the "View incidents/hazards" or "Incident Register" link (as applicable), then select the Assigned Incidents tab. Navigate to the appropriate incident or hazard you wish to complete and select the Complete Action Plan button. The Action Plan page appears and amendments to the plan can be made.

How can a control measure be marked as complete?

You can do this in two ways, depending on when the control measure was completed.

If you completed the control measure today, then just click the Completed? check box under the My Tasks section, then select the Save button.

Otherwise, from the Welcome page select either the "View incidents/hazards" or "Incident Register" link (as applicable), then select the Assigned Incidents tab. Select the Complete Action Plan button, and under the Actual Completion Date heading, select the checkbox. You can then select the Change Date link and specify the date that the control measure was completed. Select the Save icon at the top of the page.
